Agreement to Terms

By accessing or using the services provided by Empowered Therapy Collective LLC ("we," "us," or "our"), you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

Services Provided

Empowered Therapy Collective provides pediatric speech-language therapy and occupational therapy services. Our services are provided by licensed professionals and are subject to the following conditions:

• Services are provided on a scheduled appointment basis
• Therapy plans are individualized based on professional evaluation
• Services may be provided in-clinic or via teletherapy as appropriate
• Parent/guardian participation may be required for certain services

Appointments and Cancellations

We request 24 hours notice for appointment cancellations. Late cancellations or missed appointments may be subject to a cancellation fee. We understand that emergencies happen with young children and will work with families on a case-by-case basis.

Payment Terms

Payment is due at the time of service unless other arrangements have been made. For insurance-covered services, copays and deductibles are collected at the time of appointment. We accept various payment methods including credit cards, HSA/FSA cards, and cash.

Confidentiality

We are committed to protecting your privacy and the confidentiality of your child's health information in accordance with HIPAA regulations. Information will only be shared with your written consent or as required by law.

Consent for Treatment

Parent or legal guardian consent is required for all evaluation and treatment services. You will be asked to sign consent forms prior to beginning services. You may withdraw consent at any time.

Limitation of Liability

While we strive to provide the highest quality care, we cannot guarantee specific outcomes from therapy. Progress depends on many factors including the nature of the condition, frequency of sessions, and follow-through with home activities.

Intellectual Property

All content on our website, including text, images, logos, and materials, is the property of Empowered Therapy Collective LLC and is protected by copyright laws. You may not reproduce or distribute our content without written permission.

Changes to Terms

We reserve the right to modify these terms at any time. Changes will be effective upon posting to our website. Continued use of our services constitutes acceptance of modified terms.
